Media Release Acknowledgement

I hereby give UNAVSA and its trustees, officers, employees, agents, representatives, students, and volunteers, consent to use and share any of my recording, images and or likeness, both photographic and cinematic, for the purpose of The 19th Annual Union of North American Vietnamese Student Associations Leadership Conference (also referred to as UNAVSA-19 and U-19). I further understand and give consent for such content to be used in publications, electronic media, advertising, trade, and other lawful uses to further the efforts of UNAVSA. I release UNAVSA and its trustees, officers, employees, agents, representatives, students, and volunteers, from any and all liability for any rights of action or claims herein, but not limited, to any privacy rights, proprietary rights, and any rights to compensation. 

I am attesting that I have been given ample time and opportunity to read and understand and am acknowledging of my own volition.
