To our UNAVSA Constituents,

The 19th Annual UNAVSA Leadership Conference Team welcomes you to join us this summer in Newark, New Jersey for the return of our flagship conference. Our team is eager for you all to join us! With the ongoing COVID-19 pandemic, we have a responsibility to you, our constituents, to maintain a safe environment for all. We will hold everyone accountable in creating this safe space in order to preserve the integrity of our values.

At this time, the UNAVSA-19 Conference is still being held this summer. We will be adhering to these safety measures in the jurisdiction that our conference is being held in order to maintain the safety of our constituents, our staff, our guests, and the safety of those around us.

• Attendees are required to present proof of full vaccination + 1 booster shot received at the time of UNAVSA-19 Conference check-in. Full vaccination constitutes either 2 doses minimum of the Pfizer/Moderna vaccine OR 1 dose minimum of the Johnson & Johnson vaccine.
• Attendees may have a photocopy of their vaccination certificate in lieu of the actual copy of their vaccination certificate.
• Additionally, attendees are required to present a negative PCR test taken within 5 days prior to UNAVSA-19 OR a negative antigen test taken within 3 days prior to UNAVSA-19.
• Attendees that are unable to present legitimate proof of vaccination status and a negative COVID-19 test will not be allowed to participate in conference programming.

With this in mind, UNAVSA is not liable to any guest contracting COVID at the UNAVSA-19 Conference. UNAVSA is not liable to reimburse any expenses incurred due to contracting COVID at conference, causing our attendees to not be able to participate in further conference programming. UNAVSA is not liable to reimburse any expenses due to no valid proof of vaccination status. Attendees will show the registration team during check-in their vaccination card but the UNAVSA-19 staff team will not keep any further medical records of each attendee.

During Conference, the UNAVSA Conference Staff will have a small number of tests available for those who may develop symptoms while at the conference. If you develop symptoms during the conference, please notify conference executives; our attendees will be able to contact them through their respective family leaders. Immediately return to your room and our staff will come to assist you. You will be tested again with one of our available test kits. 

Waiver Acknowledgement

By registering for the Annual UNAVSA Leadership Conference 19 (UNAVSA-19) from July 11th, 2024, to July 14th, 2024, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending UNAVSA-19 and that such exposure or infection may result in personal injury, illness, permanent disability, and/or death. I understand that the risk of becoming exposed to or infected by COVID-19 at UNAVSA-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, UNAVSA staff, UNAVSA staff, and program participants and their families.

I acknowledge that we have no perfect plan to fully prevent the spread of COVID-19, and that there will always be an inherent risk with holding an in person event. UNAVSA-19 Staff will do our best to create an event that is as safe as possible.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my participation in events held by or associated with UNAVSA. I hereby release, covenant not to sue, discharge, and hold harmless UNAVSA, its volunteers, contract workers, agents, and representatives, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of UNAVSA, its volunteers, contract workers, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation held by or associated with UNAVSA. 

I agree that I comply with all health and safety rules and guidelines imposed by UNAVSA or by any state, local, or federal governmental entity, and will practice safe social distancing and clean hygiene during my participation at events held by or associated with UNAVSA.

I agree that I have had sufficient time to read this entire document prior to signing. Also, I understand that this activity might not be made available to me or that the cost to engage in this activity would be significantly greater if I were to choose not to sign this release, and agree that the opportunity to participate at the stated cost in return for the execution of this release is reasonable. I have read and understood this document and I agree to be bound by its terms.
